We’ve got a few questions that will help us to assess our suitability to support you and align you with an available therapist. This form will ask about your therapy goals, current wellbeing, previous mental health diagnoses, and offers you the opportunity to ask any questions of us. It should take around 5 minutes to complete.

After we receive this information, you will receive an email and/or call advising of our capacity to support you. If one of our psychologists has availability for sessions with you, the next steps will be discussed at this time.
